Ms. H, 29 years old, had one ovary and fallopian tube removed during her bright and youthful years before marriage due to a low malignant potential ovarian tumor (LMP). After getting married, she began IVF treatment at another medical institution. However, due to a low number of eggs and poor embryo quality, embryo transfer could not be performed.
The first failed IVF treatment left her feeling extremely frustrated. However, because her ovarian condition did not allow her much time to wait, she came to Yun-I to prepare for another IVF treatment, hoping to achieve pregnancy as soon as possible.
During the examination, Dr. Hui-Po Li found that after ovarian surgery, her ovarian function had declined rapidly. Her AMH level was only 0.84 ng/ml. The remaining fallopian tube showed signs of hydrosalpinx, and pelvic adhesions were also present. After detailed consultation and explanation by Dr. Li, the couple decided to follow the doctor’s recommendation: first taking supplements to try to improve egg quality and quantity, and then proceeding with IVF treatment.
After nearly six months of adjustment, in April 2023, Dr. Li evaluated her blood test results and follicle condition and determined that she was ready to enter treatment. After carefully selecting the medication protocol and timing for egg retrieval, Dr. Li successfully retrieved 5 mature eggs from the remaining ovary. After egg retrieval, signs of ovarian tumor recurrence were discovered, and the remaining ovary underwent another surgery in September. After the second surgery, very little ovarian tissue remained. From the perspective of reproductive medicine, Ms. H was almost no longer able to undergo ovarian stimulation and egg retrieval, making the 3 frozen embryos even more precious.
After the surgery, Ms. H rested for several months. In January 2024, Dr. Li performed thawed embryo transfer for her. After such a long and difficult fertility journey, the couple sincerely hoped that this treatment would finally bring good news. Dr. Li used hormone replacement therapy and carefully adjusted the thickness of the endometrium, making thorough preparations for embryo implantation.
Low malignant potential ovarian tumors are a very challenging condition in obstetrics and gynecology. This is especially true for young women of reproductive age, as a balance must be achieved between preserving the ovary and fulfilling fertility needs.
