48 Eggs 🥚 Egg Freezing vs Ovarian Hyper-Response
A woman who came for egg freezing had 48 eggs retrieved yesterday.
She is young, very slim, has irregular menstrual cycles, an LH/FSH ratio above 5, and an AMH level of 11.
On the second day of her menstrual cycle, ultrasound showed a very large number of antral follicles.
These conditions confirmed that she has polycystic ovary, PCO.
Therefore, on the second day of her menstrual cycle, when we began using ovarian stimulation injections, I had already informed her in advance that this could possibly trigger Ovarian Hyperstimulation Syndrome, OHSS, and asked her to take preventive measures.
OHSS refers to a stronger-than-usual ovarian response after medication stimulation, resulting in the development of many follicles.
This may lead to a series of physiological reactions such as abdominal bloating, ascites, decreased urination, and electrolyte imbalance, which then cause discomfort symptoms.
In general, OHSS is more common in women who are young, slim, have high AMH levels, and have PCO.
Therefore, during treatment, if we find that the number of follicles exceeds 15, or estrogen levels exceed 3500, we will remind the patient to pay attention and take preventive measures.
However, for those undergoing egg freezing or IVF treatment, if this type of reaction occurs, it is both good news and a concern.
The good news is that enough eggs can be retrieved in one cycle for freezing or for embryo creation, which can greatly help future pregnancy success.
The concern is that OHSS may cause discomfort, and in some cases may even require drainage of ascites or hospitalization.
However, because the medical community has developed a clear understanding of OHSS over the years, we provide patients with sufficient information so they can take preventive measures.
For example, supplementing protein and electrolytes can help.
The trigger shot may also use Decapeptyl instead of HCG to reduce the amount of HCG used and decrease the series of side effects it may cause.
If OHSS occurs, continued observation and follow-up after egg retrieval are still necessary.
